Where this came from
We spent several years building software for healthcare practices — scheduling, records, patient messaging, the whole operational stack. Across more than a thousand practices, the same pattern kept surfacing in the data and in every conversation with an owner: the front desk was the bottleneck, and everyone had quietly accepted it.
Not because the staff were not good. Because the job is structurally impossible. One person cannot check in a patient, process a payment, answer a clinical question and pick up a ringing phone at the same time. So the phone loses — and unlike a missed appointment or an unpaid invoice, a missed call leaves no record. There is nothing in any report to look at.
optni.ai is the narrow version of that problem, solved properly. Not a platform, not a records system, not a phone company. A front desk that answers.
What we deliberately don't do
We do not build an EMR. We do not touch the clinical chart. We do not give medical advice or perform triage. Every one of those is a deliberate boundary, and each one buys something real: a practice can be live in a day, the compliance surface is small enough to explain on a single page, and nobody has to migrate anything to try it.

optni.ai is a US company, built by the team behind ConnectAI, which serves over a thousand healthcare practices. optni.ai runs as its own product for the US market — its own roadmap, its own compliance posture, and its own opinion about what a front desk should do.

The agents say what they are
Ava introduces herself as an AI assistant on every call. Pretending otherwise wins a few seconds of goodwill and loses all of it the moment someone notices — and on a recorded call it creates real legal exposure.
We stay out of the chart
No EMR, no clinical records, no decision support. That boundary is why a practice can be live in a day instead of a quarter, and why our HIPAA surface is small enough to explain on one page.
Escalation is a feature, not a failure
The measure of a good front desk agent is knowing what it should not handle. Clinical questions, distressed callers and genuine emergencies go to a human, fast.
